Vehicle is roadworthy — but the defect must be reported and recorded. You can continue driving, but the fault must be documented clearly and reported to your employer or engineer without delay.

📝 How to Fill In the Form — Minor Defect
- 1 Truck Registration, Start of Duty, Date, End Tacho Start & Tacho End, and Route Details — all of these boxes MUST be filled in. Do not skip any of them.
- 2 All checked boxes must have some sort of fill — a tick, cross, or initials. Never leave a box completely empty.
- 3 If a piece of equipment is not present on the vehicle — write N/A in that box. Do not leave it blank.
- 4 Describe the defect clearly — write what the fault is, where it is on the vehicle, and how it was noticed. Be specific: vague descriptions are not acceptable.
- 5 Who was it reported to? — write the name of the person you reported the defect to (your employer or the engineer). This is required.
- 6 "Has the defect been rectified?" — answer NO (the defect has been reported but not yet fixed at time of inspection).
- 7 "Is the vehicle roadworthy?" — answer YES (the fault is minor and does not prevent safe driving).
- 8 Your name and signature — you MUST print your name and sign the form. An unsigned form is invalid.
- 9 If the same defect continues on the next day — fill in a new defect sheet the same way each time, every day, until the defect has been fully repaired and resolved.
Recurring defect? If the same defect persists across multiple days, you must fill in a new form each time using exactly the same method — until the defect is gone and confirmed repaired. This creates an audit trail and protects you legally.
