📝 How to Fill In the Form — Serious Defect
- 1 Truck Registration, Start of Duty, Date, End Tacho Start & Tacho End, and Route Details — all of these boxes MUST be filled in. Do not skip any of them.
- 2 All checked boxes must have some sort of fill — a tick, cross, or initials. Never leave a box completely empty.
- 3 If a piece of equipment is not present on the vehicle — write N/A in that box. Do not leave it blank.
- 4 Describe the serious defect in full detail — explain exactly what the fault is, where it is located on the vehicle, what you observed, and when you first noticed it. Be thorough.
- 5 Who was it reported to? — write the name of your employer. A serious defect must be reported directly to the employer (not just an engineer).
- 6 "Has the defect been rectified?" — answer NO (the vehicle has not been repaired at the time of filling in the form).
- 7 "Is the vehicle roadworthy?" — answer NO. The vehicle must not be driven until the serious fault has been fully repaired and signed off.
- 8 Your name and signature — you MUST print your name and sign the form. An unsigned form is invalid.
You are NOT permitted to take a vehicle with a serious defect onto the road. Doing so is a criminal offence. The vehicle must be taken out of service immediately and must not move until repaired and declared roadworthy by a qualified person.
